Chapter 84a — Extracorporeal Carbon Dioxide Removal

CREATED 2026-09-23 by author decision, closing the gap Chapter 27 recorded. ECCO2R had no home anywhere in the 92-chapter map. Placed in Part XII as an advanced strategy rather than in Part I as a configuration, because it is a distinct therapy with its own indications, its own trial evidence and its own failure modes, not a variant of full ECMO. NOTE: the original suggestion of 'Chapter 78' was wrong — Ch 78 is already Hybrid ECMO Configurations — so this sits at the end of Part XII as 84a (sort key 84.5), by the same convention as Ch 38a, to avoid renumbering published cross-references. Scope when drafted: the physiological separation of carbon dioxide clearance from oxygenation, low-flow versus high-flow devices, indications in hypercapnic respiratory failure and as a lung-protective adjunct, the randomised evidence, anticoagulation at low blood flow, and how it differs from VV ECMO at every level. Ch 2 owns the gas-exchange physiology it builds on; Ch 27 owns the measurement.
