Autoimmune & Rheumatology System, Protocol 1/5. Overlaps substantially with the Diffuse Alveolar Hemorrhage protocol (Respiratory System) and the AKI protocol's glomerular disorders table (Renal System) β this protocol serves as the unifying framework connecting both organ-system manifestations.
1. Definition
Pulmonary-renal syndrome (PRS) = the combination of diffuse alveolar hemorrhage (DAH) and rapidly progressive glomerulonephritis (RPGN), most commonly from ANCA-associated vasculitis or anti-GBM disease (Goodpasture syndrome). PRS is UNIVERSALLY FATAL if not recognized and treated in a timely manner β this single fact should drive the entire management posture: aggressive empiric treatment on clinical suspicion, not a leisurely diagnostic workup.
2. When to Suspect
Diffuse pulmonary infiltrates, anemia, and low-grade fevers IN THE SETTING OF hematuria, RBC casts, and acute renal failure β this combined pulmonary-PLUS-renal pattern is the trigger. The vasculitides should be suspected in ANY patient with constitutional symptoms plus kidney injury and an ACTIVE URINARY SEDIMENT (RBC casts specifically suggest a glomerular origin, not just "blood in the urine").

Differential diagnoses to actively exclude given overlapping presentation: community-acquired pneumonia with sepsis and ATN, SLE with lung involvement and lupus nephritis, sarcoidosis, and infections including leptospirosis, Legionella, ehrlichiosis (pneumonia with AIN/ATN), pancreatitis with pneumonia and ATN.

Cavitary lung lesions and saddle-nose/subglottic stenosis are relatively specific pointers toward GPA over MPA; asthma history strongly points toward EGPA; the degree of mononeuritis multiplex severity roughly tracks GPA < MPA < EGPA.
5. Investigations
Anti-GBM antibody: highly sensitive (95%) and specific (99%) for Goodpasture disease β a genuinely excellent test when positive or negative.
ANCA serology in GPA: PR3 antibody (c-ANCA) elevated in >75% of cases; ~20% have elevated MPO antibody (p-ANCA); <5% are ANCA-negative β a negative ANCA does not fully exclude GPA, though it is uncommon.
Kidney biopsy may ultimately be necessary for definitive diagnosis in patients with intrinsic renal pathology, but treatment should NOT be delayed pending biopsy in a clinically compelling, rapidly progressive presentation given the universally-fatal-if-untreated natural history.

6. Immediate Treatment β Do Not Delay for Diagnostic Certainty
Immediate IV corticosteroids: methylprednisolone 7-15 mg/kg/day and/or up to 1 g/day for 3 days, followed by oral prednisone 1 mg/kg/day (up to 60-80 mg).
Cytotoxic immunosuppression: oral cyclophosphamide 2-3 mg/kg/day, OR IV cyclophosphamide 15 mg/kg or 750 mg/m2.
In severe, life-threatening illness with strong autoimmune-etiology suspicion: therapeutic intervention should NOT be delayed β once infection has been reasonably excluded, EMPIRIC glucocorticoids can be initiated while rheumatology and other appropriate specialties are consulted in parallel, not sequentially after a full workup.
Stress-dose steroids (hydrocortisone 50mg IV q6h or equivalent) are required for any patient CHRONICALLY on corticosteroids (even if not currently) during critical illness or surgical procedures, to prevent adrenal insufficiency-related shock (see Adrenal Crisis protocol, Endocrine & Metabolic System).
7. Rituximab vs Cyclophosphamide β Trial Evidence
RAVE trial: rituximab showed SIMILAR outcomes to cyclophosphamide in NONINTUBATED patients with serum creatinine <4 β lower-quality data support rituximab use OUTSIDE these exclusion criteria as well, but the strongest evidence base is in this specific, less-severe subgroup.
MAINRITSAN trial (and long-term follow-up): rituximab is SUPERIOR to azathioprine for MAINTENANCE of remission and improves long-term survival β relevant for the post-acute, longer-term management phase once the acute crisis is controlled.
8. Plasma Exchange β Genuinely Nuanced Evidence
PEXIVAS trial: plasma exchange did NOT reduce death or end-stage renal disease incidence in the broader ANCA-vasculitis population β plasma exchange should NOT be done routinely, and risks/benefits should be weighed individually.
Lower-quality data show favorable outcomes SPECIFICALLY in patients at high risk of ESRD, those already on RRT, or those with DAH β a meaningfully different (more favorable) risk-benefit calculus in this specific high-risk subgroup, even though the overall trial result was negative.
Plasma exchange for Goodpasture disease (anti-GBM) specifically remains recommended, and one study identified therapeutic plasma exchange as the SINGLE predictive factor associated with long-term dialysis independence in ANCA-positive vasculitis with advanced renal failure β apply plasma exchange selectively per this evidence pattern (DAH, advanced renal failure/RRT, or confirmed anti-GBM) rather than as a blanket therapy.
9. Immediate Stabilization (ABCDE)
Airway/Breathing: support hypoxemic respiratory failure per standard principles and the full DAH protocol (Respiratory System) if diffuse alveolar hemorrhage is present β mechanical ventilation with lung-protective strategy as needed; active bleeding is NOT an absolute contraindication to ECMO if refractory hypoxemia develops, per the DAH protocol's cross-reference, with anticoagulation strategy modified accordingly.
Circulation: standard hemodynamic support; correct anemia from DAH-related blood loss per standard transfusion principles.
Renal: RRT per standard AKI indications if renal failure progresses (see AKI and CRRT Indications protocols, Renal System).

12. Monitoring Framework
Serial renal function, urinalysis (RBC cast trend), respiratory status per the DAH protocol, serial hemoglobin (DAH-associated blood loss), ANCA/anti-GBM titer trend during treatment.
13. Complications
Respiratory failure from DAH, ESRD/dialysis dependence, treatment-related complications (cyclophosphamide cytopenias/infection risk, high-dose steroid complications), missed diagnosis with fatal outcome if treatment delayed. Prevention: prompt empiric treatment on clinical suspicion, selective (evidence-guided) plasma exchange application, stress-dose steroid coverage for chronic steroid users. Rescue: full DAH protocol respiratory rescue therapies (including ECMO consideration), RRT escalation, rituximab/cyclophosphamide escalation per trial-guided selection.
14. Escalation & De-escalation
Escalate: DAH with refractory hypoxemia -> full DAH protocol rescue (proning, ECMO consideration); advancing renal failure -> RRT, reconsider plasma exchange given the high-risk-subgroup evidence.
De-escalate: clinical/serologic remission achieved -> transition to maintenance immunosuppression (rituximab per MAINRITSAN evidence preferred over azathioprine), taper steroids, ongoing rheumatology/nephrology follow-up.
15. ICU Discharge Criteria
Respiratory status stable, renal function stable or on an established RRT trajectory, induction immunosuppression established with a maintenance plan, hemodynamically stable, rheumatology/nephrology follow-up arranged.

19. Controversies
Plasma exchange's role remains genuinely nuanced post-PEXIVAS β a negative headline trial result coexists with favorable lower-quality evidence in specific high-risk subgroups (DAH, advanced renal failure), and practice reasonably varies in how much weight to give the subgroup signal versus the overall negative trial. The precise threshold for choosing rituximab vs cyclophosphamide as first-line induction therapy (beyond the RAVE trial's studied population) continues to be individualized based on patient factors, cost, and institutional experience.
