The learning curve in ECPR is a system curve, not an operator curve. A new programme should expect its early results to be worse and should plan volume, selection and rehearsal accordingly.

Survival improved as centre experience accumulated. The authors attribute the gain to a summation of training, patient selection and protocol standardisation rather than to cannulation skill alone.
